Let me expound on my previous post. So that people understand what’s involved
in total knee replacement I’ll leave out some of the steps that don’t lead to
post op pain and hit the highlights of some aspects that might. After opening
the joint by cutting some of the side ligaments the top of the tibia (lower
leg bone) is sawed off and a metal plate with a stabilizing post is driven or
cemented in place to hold what will be the support for a nylon like lower
cartilage replacement. The former (thigh bone) will have the prominent
cartilage areas ground off and the upper metal components pressed into the
remaining bone anchored by spikes which are part of this component. The back
of the kneecap may or may not need grinding and a nylon like replacement cap
placed. The anterior and posterior knee ligaments may need severing which can
vary based on the type of prosthetic knee used. The main ligament from the
thigh muscle and that muscle itself may need surgery to obtain proper
alignment. Then reconstruction of the capsule which is full of nerve sensation
needs to be accomplished. This is not a complete description of the surgery,
but sawed and grinding bones are not a 7 day pain event. The post operative
stretches to get mobility start on the operation day. Without this early
mobilization which continues for weeks a limited range of motion results and a
poor result occurs. Earlier restrictions make written prescriptions necessary
for narcotics so called in refills not available.
